Joseph Wong, MD, MBA
Medical DirectorDr. Joseph Wong earned his undergraduate degree at the University of Pennsylvania, his MD from Case Western Reserve University, and completed his PM&R residency at Stanford University. He holds board certifications in both Physical Medicine & Rehabilitation and Electrodiagnostic Medicine. He specializes in functional restoration and neuromuscular diagnostics in the skilled nursing facility setting, with a focus on improving patient outcomes, reducing readmissions, and supporting therapy teams through expert physiatry consultation.
Bruno Subbarao, DO
PM&R PhysicianDr. Bruno Subbarao is a Physical Medicine & Rehabilitation physician who brings a unique systems-thinking approach to patient care. He completed his undergraduate studies at New York University and earned his medical degree from Lake Erie College of Osteopathic Medicine in Bradenton. A passionate guitarist and musician, Dr. Subbarao enjoys traveling, bouldering, and filmmaking — interests that fuel his creative problem-solving in clinical settings. He is particularly drawn to PM&R because it demands coordination across specialties, integration of therapies, and a sharp focus on measurable functional outcomes. Dr. Subbarao believes the highest level of patient care emerges when clinical excellence is paired with operational excellence.
Tiffany Calloway Starks, MD
PM&R PhysicianDr. Tiffany Starks is a board-certified physician specializing in Physical Medicine and Rehabilitation. She earned her undergraduate degree from Spelman College and her medical degree from The Ohio State University College of Medicine and Public Health. She completed her residency training at the University of Pittsburgh Medical Center Physical Medicine and Rehabilitation Residency Program. Dr. Starks is deeply committed to the power of human connection in medicine. She believes that "half of life is showing up," and she chose the field of medicine because of her genuine appreciation for meaningful patient relationships and her lifelong fascination with human physiology. Outside of medicine, Dr. Starks finds joy and balance through her family life with her husband and two daughters. Her personal passions include traveling, reading, scuba diving, hiking, and attending concerts and sporting events.
Alexandria Mahoney, MSN, APRN, FNP-C
Nurse PractitionerAlexandria Mahoney earned her Master of Science in Nursing as a Family Nurse Practitioner from Northern Arizona University, her Bachelor of Science in Nursing from Grand Canyon University, and her Bachelor of Arts in Psychology from the University of Arizona. She has completed advanced training through the Institute for Functional Medicine. She previously practiced in functional medicine in Scottsdale and served as a Registered Nurse at HonorHealth Shea Medical Center for over 11 years. Alexandria specializes in holistic, root-cause care in the PM&R and skilled nursing facility setting, with a focus on restoring function and improving quality of life for medically complex patients. An Arizona native and proud mother of two girls, she enjoys an active lifestyle including the gym, hiking, and traveling.
Katie Rambo, AGACNP-DNP, BSN
Nurse PractitionerKatie Rambo earned her Doctor of Nursing Practice in Adult-Gerontology Acute Care Nurse Practitioner from the University of Arizona. She was inspired by her grandmother, who was a nurse, to pursue a career in nursing and advance to become a nurse practitioner. She specializes in acute care and post-acute rehabilitation in the skilled nursing facility setting, with a focus on managing complex medical conditions, supporting recovery, and improving patient outcomes. When not seeing patients, Katie enjoys working out, hiking with her dogs, playing pickleball, and traveling. She cherishes spending time with her husband and son.
Thomas James Hooker, MSN, FNP-C
Nurse PractitionerThomas James Hooker earned his Master of Science in Nursing as a Family Nurse Practitioner from Grand Canyon University in 2025 and his Bachelor of Science in Nursing from Arizona State University. He brings clinical experience from the Emergency Department and Cardiac Telemetry Unit, as well as hospice care. Thomas specializes in post-acute rehabilitation in the skilled nursing facility and rehab setting, with a strong emphasis on lifestyle modifications, daily exercise tailored to patients' abilities, and helping patients return to their prior baseline with improved quality of life. A proud father of two boys, he enjoys pickleball, soccer, swimming, hiking, biking, and playing Nintendo games with his family.
